Required forms
Please download a seperate packet for each camper and complete each form in the packet. A new packet needs to be completed each year for each camper. Forms from previous summers cannot be used. (Even if the information is the same). Completing these forms correctly and returning them at least 2 weeks prior to the start of camp will expedite the check-in process on opening day.
